*Please note that this form includes a request for submission of two paper copy forms-- Earlham College Risk, Release and Waiver of Liability Form for Minors Programs  and Earlham College Media Release for Minors Programs. These forms can be uploaded to the Qualtrics form directly OR turned in to Program/Event Staff on or before the start of the program/event.
