Required Forms
Front & Back Copy of Your Insurance Card, Copy of a Physical Performed Within One Year of the Start of Camp, All Medical Clearance & Waiver Forms must be completed and signed by a parent and a physician prior to the start of camp.

Teams Staying Overnight
For teams staying overnight, you will be required pay the camp tuition plus $50 per head to stay in the dorms for 1 night.

Camp Details
→ For High School Teams JV-Varsity
→ Minimum of 3 games (18 minute halves / running clock), 2 refs and 1 scorekeeper
→ NCAA Initial Eligibility Seminar
→ All team players must have high school eligibility remaining.
→ Maximum of 15 players per team
→ Teams have the option of choosing any of the 3 days for camp

Note
Online registration will terminate 48 hours prior to the beginning of each camp. You also have the option for pre-registration by mailing in a check. Teams will be notified of registration once game schedule is made.
